The idea that world design had for these was that they were equivalent to towns you find in traditional fantasy RPGs: places where you can find vendors, and chat with people without having to worry about wandering monsters.

It definitely sounds like it was smaller than expected. If combat is working, then I'd say the patch is good--I guess they just straightened something out between the initial QA server patch and this one.
