Just got this coded and am gonna test it out a bit.

I can't pin this down at all. It happens with pvp...happens on missions =/ Each time I die on a box 3 mission I have to relog....I'll try and keep an eye out for a pattern, but haven't noticed one thus far.

I have to log in and out of the game to get rid of this each time I die it seems /bug'd it...but really annoying....can't use any abilities due to 'effects of combat'

You can access that menu from about any open folder window. Just double click your 'my computer' and go from there.

If you look closely at the picture i posted, you can see you need to go to Tools and Folder Options, then View....
