Don't know if this was ever mentioned before but here goes...
I was thinking that they should make a five tier mission but with a twist. say you have a six man team, in a normal mission usually all six would go to do the 1st , 2nd and sometimes the third mission together (depending if its a two or three tier mission). My idea was the team goes out together the first two missions but for the third tier the mission leader has to split the team into two three man teams. For the third and fourth mission tiers both teams can run these at the same time and come back together for the final tier mission.
I liked the idea because I think it would increase the difficulty a bit, plus you would get everyone in the team involved. The only backfire is that most teams have drastically differing levels., so maybe something like this for Pandora mission series maybe?
