I was just randomly thinking earlier..but I know the chances of it happening are slim, but imagine the hardline was ringing for an operative to get out and a bluepill used it, would they end up in the real? or what?
I know the operator would control it but say he wasnt paying attention.

It might happen that the blue-pills mind is transferred to the operatives body similar to how Smith takes control of Bane in Reloaded.

I would assume that the computer system would recognise the signal pattern is not that of the operative and just throw up an error. The signal is always being sent to the simulation from the ship so is always connected to the operative. Bane's mind got overwritten but his signal remained the same allowing Smith to use his body.

Nothing would happen. Each RSI contains an input/output carrier signal, which functions as both an IP and password. When the Hardline rings as an exit, it's basically asking for a login. The Bluepill's input/output carrier signal does not match that of any of the operatives jacked in, so it gets rejected, and they probably just hear a dial tone.
