After several instances i pieced together a theory over why this was happening.
While running a mission where in which a neccesarry item is dropped ie. a key, frag, disk etc. if your inventory is full you will not recieve the item instead it will be deleted causing an abrupt mission failure.
I do believe the cause of this is based on the luggable system - only items that are luggables can be dropped ergo this nothing items are deleted as opposed to the way they used to be dropped by NPC's when your inventory was full.
I don't know if perhaps it's something greater than that, but then it should be relatively easy to fix since it can use the same code or logic of decompiled frags dropping to the ground around your feet or at the very least of regular items that stay within the corpses inventory.
In either case it's just another note for us as players to make sure our inventories don't fill up - but when your grinding like crazy they tend to stack up while you just keep going.
