I'm pretty sure they are still available at any item vendor in Richland at least. Then you just right click and say either "use" or "activate item" and it takes away your Attributes and you can reset them as you please. And, if they aren't still at the item vendors, they are at your Archivist Vendors. Meaning, the guy that sells you the books to get into the Archives. Yeah, he's got a Stat Hack Ticket which gives you a mission to get one.
Be warned, though. The mission is not hard, but very VERY tedious.
Edit: Forgot to mention, this is purely on the Test Server. There is currently no way to get one on the Live servers.
